The Personalised Eye Care Toolkit was created on the basis of the Personalised Healthcare Hub and Toolkit for the Patient Community and was co-created with patient community leaders. It includes a suite of educational resources on the topic of personalised eye care and its benefits for people and communities affected by retinal diseases.
The toolkit is for patient advocacy groups, patient community leaders and people living with low vision and their supporters. The resources have been created in a variety of formats so you can use them in different situations, such as meeting and event presentations, handouts and as conversation starters. The toolkit has been adapted to people living with low vision.
